29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 5010 Vaughn Road, Montgomery AL; On or about October 21, 2014 the employer exposed employees to worksite hazards in that the employer failed to instruct his employees in the recognition and avoidance of unsafe conditions applicable to their work environment to control or eliminate any hazards or exposure to illness or injury. Employees were allowed to work at height without first being trained in the requirements of fall protection such as body harness and lanyard.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) 5010 Vaughn Road, Montgomery AL; On or about October 21, 2014 the employer exposed his employees to fall hazards in that employees were allowed to work 10 feet above the ground from an articulated boom lift without fall protection (safety harness and lanyard).
