29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 4, 2014, at the Las Tiendas de Zaragoza parking lot, the employer did not provide hazard recognition training for employees performing activities including but not limited to working from an over-center aerial device (boom truck) without any form of fall protection and was exposed to a fall hazard greater than 6-feet.
