29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system:     On or about November 04, 2014, and at times prior thereto, an employee was working on the roof of a commercial building without any form of fall protection. The employee was exposed to fall hazards of greater than six feet to the ground below.

The employee was exposed to fall hazards of greater than six feet to the ground below.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed:  On or about November 04, 2014 and at times prior thereto, an employee was using half of a thirty two foot ladder to access the roof of a commercial building. This exposed the employee to fall hazards of greater than six feet to the ground below.
