29 CFR 1926.703(a)(1): Formwork shall be designed, fabricated, erected, supported, braced and maintained so that it will be capable of supporting without failure all vertical and lateral loads that may reasonably be anticipated to be applied to the formwork:    (a) Concrete Frame Associates, LLC at 1390 Glenarm Place, Denver, CO 80204: On and before November 4th, 2014, the employer did not ensure that employees were protected from struck-by hazards when rolling a 6,400 pound concrete floor section, supported by metal trusses, with the trusses set on four 410 high metal roll stands, when the horizontal load caused the roller stands to tip over, in the following instances:  1) The four metal roller stands had a small base and top (16 x 18), and the truss stands were not designed and certified to support the horizontal loads placed on them;  2) These employees were not trained on safe procedures on the use of the truss roller stands; and  3) The truss roller stand setup for this concrete truss was not inspected by a competent person prior to and during use.  Condition exposed employees to stuck-by hazards caused by the truss roller stands tipping over.      Abatement note: Among other methods, feasible and acceptable abatement methods are stated as follows:  1) The TJF Structural Design, Inc. directions for use of these truss roller stands provided that the horizontal load shall be limited to 5% of the vertical load.  The specifications also provide that the frame stability should be checked when the towers are not perfectly aligned;  2) The American National Standard Institute (ANSI) and American Society of Safety Engineers (ASSE) A10.9  2013 provides in Section 4.10: Training.  Each employer shall ensure that each employee has been provided training and evaluation for the work and avoidance of hazards specific to the activities involved with and related to masonry, concrete placement, precast placement, reinforcing steel activities, post-tensioning, rigging, placement of shores and re-shores, formwork and handling of chemicals such as cement, concrete, and form-oils.;  3) ANSI/ASSE A 10.9  2013 Section 4.4.11 provides that, A competent person shall conduct field reviews to verify that the plan is being implemented as approved and that the hazards have been identified and corrected&  The competent person was responsible for testing the tall roller stands and considering the use of outriggers to support the horizontal loads.
