29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Building Exterior - Employees were exposed to head injuries while working below masonry activities. The employees were not protected by hard hats or protective helmets, on or about October 17th, 2014.

A) Building Exterior: Employees were using a flexible extension cord that was frayed and damaged, on or about October 17th, 2014.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    A) Building Exterior: Employees were exposed to fall hazards of up to approximately 12 feet while working on a fabricated framed scaffold that was not provided with safe access, on or about October 17th, 2014.

A) Building Exterior: Employees were exposed to fall hazards of approximately 12 feet while working on a fabricated framed scaffold that was not protected by a complete guardrail system or other means of fall protection in place, on or about October 17th, 2014.
