On or about November 5, 2014, at the south side of the roof, an employee operating a portable circular saw to cut plywood was not wearing eye protection being exposed to struck-by hazards from flying debris.

On or about November 05, 2014, two employees performing framing work on a 6:12 pitch roof were exposed to an 11 ft. 1 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system.
