29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.      (a)  All Needs Welding, at the 3400 block of W. Clyde Pl., Denver, CO 80211: On and before November 3, 2014, the employer did not ensure that each employee was instructed in the recognition and avoidance of unsafe conditions associated with transporting a steel balcony via the use of a rough terrain forklift on an active and congested residential street.  In the course of moving a steel balcony from a staging area in a near-by parking lot to the construction site an employee who was handling a tag line was run over by the right-front wheel of the rough terrain forklift.  This condition exposed employees to struck-by and crushing hazards.       Abatement Note:  Feasible abatement methods include but are not limited to:                  1) Develop a Job Safety Analysis (JSA) and review it prior to performing the movement of the balconies.          2) Determine an alternative method for transporting the balconies (i.e. loading them onto a flat-bed truck for transport).
