29 CFR 1910.23(b)(3):  Every window wall opening at a stairway landing, floor, platform, or balcony, from which there was a drop of more than 4 feet, and where the bottom of the opening was less than 3 feet above the platform or landing, was not guarded by standard slats, standard grill work (as specified in paragraph (e)(11) of this section), or standard railing:    (a)  On or about November 3, 2014, and at times prior, employees were exposed to falls of greater than 5 feet on top of the drain station where the spaces in front of and behind the cars was not guarded.
