29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) At the site, on November 4, 2014, the platform used to lift an employee approximately 14 feet was not attached to the forks of the Sky Track JLG (model number 100054, serial number 0160051158). The forklift was moved horizontally while an employee was on the platform.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) At the site, on November 4, 2014, employees pulling cable from an elevated platform supported by the forks of a Sky Track JLG (model number 100054, serial number 0160051158), approximately 17 feet above the ground, were not protected from falling by any type of a fall protection system.
