29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. (a) Front side, new townhome construction located at 960 Thibideau Court, Sandy Springs, GA 30350. The employer allowed employees to work on a tubular welded scaffold system which was approximately 26-feet without tying the system into the structure. Employees working at heights of approximately 20-feet, 11-inches were exposed to potential tipping and collapse of the system as it was not secured prior to the start of work.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. (a) Front side, new townhome construction located at 960 Thibideau Court, Sandy Springs, GA 30350: Employees installing bricks from a tubular welded scaffold system were exposed to fall hazards of approximately 20-feet, 11-inches while climbing the cross bracing and scaffold frames when a ladder was not provided for safe access to the work platform. Luis Eduardo Alvarado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 811201, citation number 1, item number 2 and was affirmed as a final order on 03/04/2013, with respect to a workplace located at 637 Concord Lake Circle, Smyrna, GA 30082.
