29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 5, 2014, at the above addressed jobsite, the employer did not affix guards on a bridge saw, that has rotating saw and a nip point, which is used by employees to cut granite slabs during granite countertop fabrication.

On or about November 5, 2014, at the above addressed worksite, employees were performing granite countertop fabrication tasks in a facility where dust, containing silica, covered work surfaces, eating surfaces, and equipment.
