The employees were exposed to electrical shock hazards.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:  On or about November 04, 2014 and at times prior thereto, extension cords were being used at the worksite and at least one cord had the ground pin missing. Employees were exposed to electrical shock hazards.

Two employees were on top of the frame without using any form of fall protection. The employees were exposed to fall hazards of greater than six feet to the ground below.
