29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) Left Side of the Building - on or about November 5, 2014 - a guardrail system was not installed on the front section of the field-fabricated scaffold platform.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Left Side of the Building, Second Story Floor - on or about November 5, 2014 - no fall protection was available for the employee that was removing debris at a door opening.  Elevation to the ground was approximately 34 feet, 6 inches.

29 CFR 1926.502(b)(2):  Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:  (a) Center Stairwell - on or about November 6, 2014 - the midrail that was installed for this opening was not attached to the right side of this opening.  Elevation to the lower level was approximately 8 feet, 10 inches.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Left Side of the Building, Second Story Floor - on or about November 5, 2014 - the employer did not provide comprehensive fall protection training to employees that were performing carpentry and masonry work on site.  Employees were exposed to fall hazards ranging between 8 feet, 10 inches to 34 feet, 6 inches.
