29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. On or about January 8, 2014, the employer did not have an established program consisting of an energy control procedures, employee training and periodic inspections for machines or equipment; such as but not limited to: the web offset printing press and sheetfed offset printing press equipment, thereby exposing employees to unexpected energizing, startup or release of stored energy  equipment hazards.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a. On or about November 5, 2014, the ingoing nip point at the paper in-feed of the Rotoman-M, web-press, Printing Unit #5, was not guarded and resulted in an employee sustaining crush type injuries to the fingers of the left hand.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts:    a. On or about January 8, 2015,  the guards which protect the rotating parts of the conveyor belt and the point of operation saddle stitcher assembly for the Muller-Martini Minuteman Stitcher Trimmer machine had been removed and exposed employees to caught-in, stuck-by and amputation hazards.    The employer fails to assure effective protection against caught-in, stuck-by and amputation hazards as required by the standard, as demonstrated on January 8, 2015, when an employee was operating the Muller-Martini Sticher Trimmer machine with the conveyor belt and saddle sticher assembly guards removed at 4250 St. Augustine Road, Jacksonville, Florida 32207.    The Hartley Press was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315482729, citation number 1, item number 1 and was affirmed as a final order on January 18, 2012, with respect to a workplace located at 4250 St. Augustine Road, Jacksonville, Florida 32207.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:  a. 29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a. On or about January 8, 2015, printing press employees used hazardous chemicals such as, but not limited to inks, cleaning and conditioning agents, compressed gases, and printing plate solutions and finishers while working in the press rooms and the employer did not provide an explanation of the new labeling system and safety data sheet.
