29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about at 11/5/14 located at 4030 Candler Walk, Cumming, GA:  The employer did not perform frequent and regular inspections where employees were installing plywood on the roof without fall protection and no safety glasses. Employees were walking under work going on overhead without hard hats. Employees were using a ladder that was only 18 inches above the landing edge. Employees were using a make shift scaffold without any rails.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about at 11/5/14 located at 4030 Candler Walk, Cumming, GA: Employees were framing a house with lumber and plywood while on the first floor roof, as scrap plywood was observed falling to the ground as employees was working below tossing up scrap lumber to the 2nd floor, without hard hats exposing them to a struck by hazards.

On or about at 11/5/14 located at 4030 Candler Walk, Cumming, GA:  Employees were observed installing plywood and cutting lumber on a house while using a pneumatic nail gun and the skill saw without safety glasses.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 09/03/14 located at Lot 108, 4270 Hammond Bridge Dr., Suwanee, GA:  Employees were exposed to falls and struck- by hazard of 17 feet from the lower level, while preforming framing work from the make shift scaffolds. The employer failed to install guard rails and it was not fully decked.

On or about at 11/5/14 located at 4030 Candler Walk, Cumming GA: Employees installing plywood on the 1st  floor  roof while working from an unprotected roof  were exposed to fall hazards of approximately up to 18 feet.
