29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    On or about November 4, 2014, at the above addressed jobsite, the employer did not establish a written respiratory protection program for employees who were required to wear an N95 filtering face piece respirator to protect them from concrete and asphalt dust particulates generated during concrete and asphalt cutting operations.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA:    On or about November 4, 2014, at the above addressed jobsite, employees were exposed to struck by hazards while performing traffic control signal loop sensor installation operations in and near vehicular traffic without providing signage indicating how motor vehicle traffic can move through the temporary traffic control zone as required by MUTCD Section 6G.03.    Virgil Cook & Son, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.200(g)(2), which was contained in OSHA inspection number 314041336, citation number 1, item number 1 and was affirmed as a final order on January 12, 2011, with respect to a workplace located at Caterpillar Drive @ Galena Road, Montgomery, IL 60538.    No abatement certification or documentation is required for this item.
