29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l), which includes the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: 120018 21st Ave., Scottsbluff, NE 69361 (Pit Area): The employer did not ensure that employees had proper documentation of forklift training as required by paragraph (l). Retraining for an employee who was involved in a forklift incident did not have documentation provided by the employer. Abatement certification is required for this violation. Abatement certification sheet is enclosed with the citation.
