29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about November 5, 2014, at the above addressed jobsite, in the Maintenance shop area, where at least three(3) employees using a drill press were not protected from contacting the drill bit. (b) On or about 18 September, 2014, at the above addressed jobsite, in the Maintenance shop area, where at least two (2) employees using a milling machine were not protected from contacting rotating parts. (c) On or about 18 September, 2014, at the above addressed jobsite, in the Maintenance shop area, where at least two (2) employees using an engine lathe machine were not protected from contacting rotating parts.

29 CFR 1910.1200(f)(8): The employer shall not remove or deface existing labels on incoming containers of hazardous chemicals, unless the container is immediately marked with the required information: On or about November 5, 2014, at the above addressed jobsite, the employer did not ensure that the containers used for the degreaser were properly labeled with the appropriate contents of each container, exposing the employees to inhalation and absorption hazards.
