29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Instance (a) Location: Western Avenue, Allston, Massachusetts jobsite. On or about November 5, 2014, an employee was exposed to a cave-in hazard while working in a six (6) foot deep trench where cave-in protection was not used. Instance (b) Location: Western Avenue, Allston, Massachusetts jobsite. On or about November 5, 2014, an employee was exposed to the hazard of a cave-in an entrapment where no safe egress, such as a ladder, had been provided by the Respondent.
