29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: Located at Harrison Beverage, Pleasantville, NJ: On or about 11/5/14, the employer did not record the following workplace injuries and illnesses on the OSHA 300 log for the calendar year 2014; a) On or about 7/21/14, Truck Driver An employee lost time and was placed on restricted work activity after lifting a keg and injuring their back. b) On or about 7/29/14, Truck Driver An employee received stitches due to a laceration on the right eyebrow after being struck by an empty keg. c) On or about 9/10/14, Forklift Operator An employee lost time and was placed on restricted work activity after injuring foot while stacking cases of beer. d) On or about 9/15/14, Forklift Operator An employee lost time and has not returned to work after an injury that occurred while lifting boxes. e) On or about 10/3//14, Truck Driver An employee received injections of Lidocaine and Kenalog after receiving an injury while pulling the hand truck.
