29 CFR 1910.147(c)(2)(ii): The employer did not ensure that an energy isolating device was used. The employer's energy control program under paragraph (c)(1) of this section was not utilized and the employer could not demonstrate that the utilization of a tagout system would provide full employee protection as set forth in paragraph (c)(3) of this section. This violation was discovered on or about November 6, 2014; where employees in the maintenance department and machine operators were required to perform service and routine maintenance work on machines and equipment, but the employer did not require LO/TO be conducted. Employees were exposed to amputation hazards in the event of accidental start-up.

29 CFR 1910.212(a)(1): The employer did not ensure that at least one or more methods of machine guarding was provided to protect the operators and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. This violation was discovered on or about November 6, 2014; where employees operating the CNC Spring Makers were exposed to amputation hazards. Machine guarding was not in place.
