Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling: Jogger table in the mailroom, between press #5 and press #6 on the Southwest side, on or about November 6, 2014 and at times prior thereto, employees were exposed to an electrical hazard while operating a Syntron J-50 Jogger that was connected to a 120V electrical gang box that was not installed in the wall per manufactures specifications.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter were not effectively closed: Conveyor press #10 in the mailroom, on the Southwest side of the conveyor disconnect, on or about November 6, 2014 and at times prior thereto, employees were exposed to a 480V electrical hazard while operating the conveyor press #10(make model not found) that had an open knockout on the electrical disconnect box.
