29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck, the employer did not ensure that each operator has successfully completed the training required by this paragraph (l). On or about 5 November 2014, ten employees were allowed to use the forklift on site without being fully trained and evaluated, exposing the employees and those around them to the hazard of being struck by the forklift.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 25 November 2014, the rotating chuck on the Wilton Press was not guarded, exposing employees to hazards such as those created by being caught in between the point of operation and the material.
