29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  JOBSITE:  A lot of broken pieces of wood and other debris were accumulated and scattered around in between two (2) ladders supporting the scaffold platform and other working/walking areas, creating trip and fall hazards. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  JOBSITE:  The employees, performing roofing activities from the platform of a ladder jack scaffold, which was erected at a height more than 18 feet from the ground were not provided with appropriate and safe means of access.

JOBSITE:  The ladders, used to support the platform of the ladder jack scaffold, from which employees were performing roofing activities were not set up appropriately to meet the 4:1 ratio between the working length of the ladders and horizontal length from the top support to base/foot of the ladder.

All Seasons Restoration & Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard {29 CFR 1926.501(b)(13)}, which was contained in OSHA Inspection Number 913026, Citation Number 01, Item Number 002a and which was affirmed as a final order on 09/17/2013, with respect to a workplace located at 330 Sound Beach, Old Greenwich, CT 06870.
