29 CFR 1910.1000(c): The employer did not ensure that an employee's exposure to silica, in any 8-hour work shift of a 40-hour work week, shall not exceed the 8-hour time weighted average limit given for silica.   a)Sand Blasting area: On or about November 6, 2014 and at times prior thereto an employee was exposed to 0.433 milligrams per cubic meter of air (mg/m3) of silica, which is 1.65 times the calculated Time Weighted Average (TWA) PEL of 0.263mg/m3.Results were calculated for 224 minutes sampling period.  The employee was exposed to silica while conducting sand blasting operations.   b)Workshop area: On or about November 6, 2014 and at times prior thereto an employee was exposed to 4.3 mg/m3 of silica, which is 1.59 times the calculated TWA PEL of 2.7mg/m3. Results were calculated for 213 minutes sampling period. The employee was exposed to silica while conducting stone cutting operations.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) Sand Blaster: On or about November 6, 2014 and at times prior thereto the employer exposed an employee to noise levels of 160.6% dose (equivalent to an 8 hour time weighted average (TWA) of 93.4 dBA) which is 1.606 times the permissible exposure limit of 100% (equivalent to an 8 hour TWA of 90 dBA) while sand blasting at the workshop area. Results were calculated for 289 minutes sampling period and included a zero increment for 191 minutes not sampled. The employer did not administer a continuing, effective hearing conservation program.
