29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   In the following instances, flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   a)    Sabre Gel Finish & Repair - The TPI industrial fan electrical cord had no strain relief between the flexible cable insulation and plug.   b)    Woodworking Shop, Grizzly Industrial Bandsaw - There was no strain relief between the anti-restart device and the flexible cable insulation coming from the bandsaw.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures were not  permanent, continuous, and effective.   In the following instances the path to ground was not permanent, continuous, and effective:   a)  34 Deck Lamination - Due to the missing grounding pin, the path to ground was not permanent, continuous, and effective for the orange extension cord used to charge cell phones and a radio;   b)  Assembly Line 2 - The Ridgid miter saw did not have an effective path to ground; and   c)  Welding Shop - The Millermatic 250X CV-DC Welding Power Source Wire Feeder did not have an effective path to ground.   North End Composites, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.304(f)(4), which was contained in OSHA inspection number 315572289, Citation 2, Item 1 and was affirmed as a final order on 06/07/2011, with respect to a workplace located at 23 Merrill Dr., Rockland, ME.
