OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in-between hazards.  On or about November 5, 2014, adjacent to the airplane hanger at 782 Jasmine Rd., Crowley, LA. an employee was exposed to a crushing hazard when he was directed to unchain the connection between the bucket of a Link-Belt 210 X2 excavator and a frac tank.  At the same time as he stood between these two objects a manager approached the cab of the excavator to have a discussion with the operator.  The excavator was not de-energized or disabled.  As a result, when the operator in the cab stood up and accidentally made contact with the joystick controller the boom extended and crushed the employee between the bucket and frac tank.

On or about November 5, 2014, adjacent to the airport hanger located at 782 Jasmine Rd., Crowley, LA, the employer failed to conduct atmospheric testing before an employee entered into a frac tank to perform cleaning of drilling fluids.
