29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about November 07, 2014, employees were exposed to falls greater than 6 feet to the ground level below, when using a 20 feet Werner extension ladder and the side rails were not 3 feet above the landing, while accessing and egressing the roof of the residential home having an 8:12 roof pitch.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about November 07, 2014, employees were exposed to fall hazards greater than 6 feet to the ground level below, when working on the roof of a residential home, not wearing safety harnesses for fall protection, while installing the asphalt shingles to the roof decking with a roof pitch of 8/12. The Abel Jaramillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 974822, citation number 1, item number 1 and was affirmed as a final order on 06/20/14, with respect to a workplace located at 7553 Copper Meadow, Converse, TX 78109.
