29 CFR 1926.405(d): Panelboards mounted in cabinets, cutout boxes or enclosures designed for the purpose were not dead front:  On or about October 30, 2014, at 3879 Peachtree Road, Brookhaven, Georgia:  Two employees performing HVAC work were exposed to electrical shock and electrocution hazards when they used an electrical outlet to power a battery charger that was underneath a 220 volt electrical panel that was not dead front.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about October 30, 2013, at 3879 Peachtree Road, Brookhaven, Georgia:  Two employees performing HVAC work were exposed to electric shock hazard when they used a Rigid Model 3002, 120 volt, 9 amp, Sawz all with strain relief damage to the cord powered by an extension cord with strain relief damage.
