29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): Within the attic of a residential home, on or about November 07, 2014, employees were exposed to the hazards of eye/skin and respiratory exposures while removing fiberglass insulation, a hazard assessment for personal protective equipment was not conducted for items such as but not limited to protective suits, respirators, eye glasses and gloves.
