29 CFR 1910.22(b)(1): Where mechanical handling equipment is used and wherever turns or passage must be made, every floor, working place, aisle and passageways shall be kept clear and in good repairs, with no obstruction across that could create a hazard.      (a)Caliber Collision Centers @ 7880 Washington Street, Denver, CO 80223:  On and before November 6th, 2014, the employer did not ensure that the concrete floor in the auto body shop was adequately leveled off to prevent a tripping/falling hazard. The concrete floor was approximately ½ inch higher at the joint section where rolling tool carts, and other mobile equipment on caster wheels had to be pushed or pulled over the raised concrete floor by employees. This condition exposed employees to a tripping/falling hazard.      (b)Caliber Collision Centers @ 7880 Washington Street, Denver, CO 80223:  On and before November 6th, 2014,  as part of the injury/accident prevention responsibilities, the employer did not ensure that the floor drain metal cap in the auto body shop was level and did not create a tripping hazard. The floor drain metal cap was approximately ½ inch above the floor concrete where employees were performing auto body installations. This condition exposed employees to a tripping/falling hazard.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    (a)Caliber Collision Centers @ 7880 Washington Street, Denver, CO 80223: On and before November 6th, 2014, the employer did not ensure that each employee who was allowed to operate the Sit-Down Clark Forklift was trained in the safe operation of Powered Industrial Trucks as required under the standard. Two employees operated the Clark Forklift in the facility without the operators training certification. This condition exposed employees to struck-by hazards from improper forklift operations.       Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.
