29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) At the jobsite, on or about 11/4/14: An employee was working from an aerial lift, elevated to approximately 14 feet from ground, without being attached to the boom or basket. General Interior Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 312374804, citation number 2, item number 2 and was affirmed as a final order on 9/28/12, with respect to a workplace located at 990 Rt. 5&20, Geneva, NY. Abatement certification must be submitted for this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) At the jobsite, on or about 11/4/14: An employee climbed from the basket of an aerial lift onto a wide parapet he was engaged in sheathing, approximately 20 feet from ground. The employee was wearing a harness with an attached lanyard and was not attached to the aerial lift boom or basket and did not attach when he was working on the parapet and outside the lift basket. General Interior Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(1), which was contained in OSHA inspection number 312374804, citation number 2, item number 1 and was affirmed as a final order on 9/28/12, with respect to a workplace located at 990 Rt. 5&20, Geneva, NY. Abatement certification must be submitted for this item.
