29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent):     A)    Grafton Ready-Mix, Inc., located at 1155 Elm Street, Grafton, OH 44044: On or about October 2, 2014, the employer did not ensure that the runway, adjacent to the aggregate conveyor, was equipped with standard railing consisting of top rail, which has a vertical height of 42 inches nominal from the upper surface of top rail to the runway, intermediate rail at approximately halfway between the top rail and the runway, and posts. Employees were exposed to a fall hazard of up to 40 feet from the runway to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:     A)    Grafton Ready-Mix, Inc., located at 1155 Elm Street, Grafton, OH 44044: On or about November 6, 2014, the employer did not ensure that performance of employees, operating powered industrial vehicle Clark, was evaluated at least once every three years, exposing employees to struck-by, caught-in-between, and tip-over hazards in a case of unqualified operator is not operating forklift in a safe manner.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have load engaging means fully lowered:    A)    Grafton Ready-Mix, Inc., located at 1155 Elm Street, Grafton, OH 44044: On or about November 6, 2014, the employer failed to ensure that load engaging means of the Clark powered industrial vehicle were fully lowered when the forklift operator dismounted the forklift and was working in its immediate proximity. Employees were exposed to lacerations, fractures, and broken bones hazards.    No additional abatement information is needed for this item.
