Texas Roadhouse Holdings, LLC, 7475 Peach Street, Erie, PA  16509, kitchen area, on or about November 1, 2014 and times prior thereto:  Employees received burn injuries on multiple occasions when a filter basket from a Newco NKT3-NS1 tea brewing machine fell into the hot tea bin causing the hot liquid splash out and burn the employees. The burn accidents and associated injuries were caused by factors that included but were not limited to an improperly sized filter basket.  The tea brewing machine was not removed from service after the accidents, tagged out of service and/or rendered safe through other equally effective methods such as moving the machine to a secure location to prevent its use.
