29 CFR 1926.652(a)(1): Each employee in a trench or excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) or (c) of this section. This violation occurred on November 7, 2014 and at times prior thereto. Employees worked at the base of a trench that was approximately 6 feet 6 inches in depth (vertical walls) installing 12 inch water pipe. Employees were not protected and were exposed to cave-in hazards.
