29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:     (a) On or about October 23, 2014, and at times prior, Bradford Auto Body, Inc. did not provide employees with training on the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting. Employees were expected to use fire extinguishers in case of small fires at the facility. A compressed air nozzle in the automobile service shop was observed with a tip that allowed air pressure in excess of 79 p.s.i., exposing employees to air injection hazards. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws:     (a) On or about October 23, 2014, and at times prior, Bradford Auto Body, Inc. did not ensure that flexible cord and cables were connected to devices and fittings so that strain relief was provided to prevent pull from being transmitted to joints and/or terminals, exposing employees to contact with electrical hazards:     1. A 110V "Thunderbolt" battery charger was observed with severe strain on its cord at the point where the cord enters the plug.     2. A 110V  "Eagle II" spot welder was observed with severe strain on its cord at the point where the cord enters the machine outer case.
