29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    The employer did not require employees to perform lockout/tagout procedures during setup and maintenance operations.  Employees' fingers, hands and arms were exposed to crushing hazards from unexpected start-up of equipment.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees assigned to perform servicing and maintenance on rubber mills were exposed to injury from unexpected start-up.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.216(f)(2): Stopping limits for mills. All mills irrespective of the size of the rolls or their arrangement (individually or group-driven) shall be stopped within a distance, as measured in inches of surface travel, not greater than 1 1/2 percent of the peripheral no-load surface speeds of the respective rolls as determined in feet per minute.    The rollers for a large friction roller rubber mill did not stop within a distance 1 to 1/2 percent as required in feet per minute. Employees were exposed to amputation from in-running nip point hazards at the bite between rollers.    Corrected During Inspection

29 CFR 1910.216(b)(1)(iii): Safety tripwire cable or wire center cord. Installed in the front and in the back of each mill and located within 2 inches of a vertical plane tangent to the front and rear rolls. The cables shall not be more than 72 inches above the level on which the operator stands. The tripwire cable or wire center cord shall operate readily whether cable or cord is pushed or pulled:    Safety trip wires for a rubber rolling mill were positioned beyond the maximum of 2 inches from the face of the rollers. Employees were exposed to amputation from in-running nip point hazards at the bite between the rolls.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable.    1. The first line of hydraulic molding presses were not equipped with catch basins thereby allowing water to accumulate on the floor behind the presses creating slip and trip hazards.    2. The second line of hydraulic molding presses were not equipped with catch basins thereby allowing hydraulic oil to accumulate on the floor behind the presses creating slip and trip hazards.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
