29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is not fully planked:     The employer is failing to protect employees from potential fall hazards associated with working from a scaffold platform less than 10 to the ground below.  Employees were using a tubular framed scaffold which was not fully decked or planked at all working levels.  This exposes the employees to potential fall hazards.

Employees were conducting work off of a tubular framed scaffold which is missing base plates and mudsills from the legs of the scaffold.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      Workers were observed re-roofing the roof of a residential style construction site (house) not wearing harness or lanyard.  The workers were exposed to falls of over 10 feet to sidewalk, dirt and stones below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.     Employees had not been trained on the proper use of portable ladders.  Multiple hazards involving the set-up and usage of the ladder on site were identified during the inspection.  These hazards and the lack of training exposed the employees to a fall hazards in excess of 9 feet to the asphalt on this site.
