Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about November 7, 2014, employees were performing roofing activities on the roof of a fire station without the use of fall protection.  Employees were exposed to the hazard of falling approximately 9 feet to the ground below.
