Location: 111 E. 48th St., New York, NY. a) Employer did not provide a flagman while employees dumped debris into a garbage truck located at street level which had heavy vehicular and pedestrian traffic. Employees were exposed to a potential struck-by hazard; on or about 11/7/14.
