29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about November 7, 2014, on the west side of the construction site, employees engaged in decking activities used pneumatic nail guns and were not wearing eye and face protection exposing the employees to struck-by hazards and eye injuries.

On or about November 7, 2014, on the west side of the construction site, employees installing decking material used an extension cord with a missing ground pin exposing the employees to electrical hazards.
