29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about November 7, 2014, on the west side of the work site, employees engaged in construction activities utilized an extension cord that was missing the ground pin to power hand operated saws exposing employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about November 7, 2014, on the west side of the work site, employees engaged in construction activities utilized an extension cord that had the outer sheathing pulled away exposing the internal wiring. The employees were exposed to electrical shock hazards.
