29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  a)  At 711 Vandenburg Avenue in Whiteman AFB, MO 65305 - Employees were hoisting roofing materials, hand tools, and other miscellaneous items to the roof top using nylon rope at the edge of the roof, approximately 22 feet above the lower concrete/earth surface, without being protected against elevated fall hazards by guardrails or personal fall arrest systems.   NOTE: IN ADDITION TO ABTEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) At 711 Vandenburg Avenue in Whiteman AFB, MO 65305 - Employees were working on a low-slope thermoplastic polyolefin (TPO) roof, approximately 22 feet above the lower concrete/earth surface, completing "punch list" tasks such as, but not limited to, sealing seams on the TPO sheets, heat welding couplings along the parapet walls, and other tie-in tasks throughout the entire portion of the roof, including roof edges, without being protected against falls.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1052(a)(3): Riser height and tread depth were not uniform within each flight of stairs, including any foundation structure used as one or more treads of the stairs:  a)  At 711 Vandenburg Avenue in Whiteman AFB, MO 65305  - Employees were removing and replacing a low-slope (TPO) roof at approximately 22 feet above the lower concrete/ earth surface, using double-stacked cinder blocks (each approximately 7 3/4" in height) on the roof to access a 30" parapet wall leading to a ladder, creating approximately 14 1/2" for the third step, exposing employees to trip/elevated fall hazards.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(9): The area around the top and bottom of ladders was not kept clear:  a)  At 711 Vandenburg Avenue in Whiteman AFB, MO 65305  - Employees were using a "Werner" 32' aluminum extension ladder to access a low-slope roof, approximately 22' above the lower concrete/earth surface, where the area around the top of the ladder was cluttered with nylon rope, extension cords, and other miscellaneous wires, creating trip and elevated fall hazards.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
