The ladder and scaffold were not secured against movement. Employees were exposed to 10 ft fall hazard. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked "Defective, Do not Use" and withdrawn from service until repaired:      a)  The employers aluminum extension ladder had damage near the bottom of the side rail, which was repaired by screwing metal to the side rail. The employer did not provide assurance that the repaired side rail remains at least as strong as the original ladder design.        Location: 9013 4th Ave, Brooklyn N.Y. 11209      On or about 11/4/2014       Abatement note: standard 1053(b)(18) requires that ladder repairs shall restore the ladder to a condition meeting its original design criteria, before the ladder is returned to use.
