29 CFR 1926.600(a)(3)(ii): Equipment parked on an incline did not have the wheels chocked and the parking brake set: a) At 225 Grey Rock Road, Midland, GA- The employer was using a Ford 550 truck (VIN: 1FDAF57F21EB77806) with a mounted ground rod driver on an inclined hill without having the wheels properly chocked, causing the vehicle to roll back. Employees were exposed to struck by hazards. An employee was fatally struck and rolled over by the front left tire of the truck.

29 CFR 1926.601(b)(14): All vehicles in use were not checked at the beginning of each shift to assure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage which could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices. a) At 225 Grey Rock Road, Midland, GA- The employer was using a Ford 550 truck (VIN: 1FDAF57F21EB77806) with a broken emergency brake handle while installing ground rods.
