OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from falling as a result of the failure of the aerial lift operating cables. a) Job site, St. Gregory's Cemetery, Zelienople, PA: On or about November 12, 2014, during tree trimming and removal activities, the employer did not ensure that an appropriate preventive maintenance program was in place for the aerial lift unit, nor that appropriate maintenance procedures were followed in accordance with the manufacturer's instructions.
