29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     The employer failed to ensure procedures were not developed, documented and utilized for the control of potentially hazardous energy:       a) This violation occurred on November 5, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 24 inch Stanat Slitter machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 24 inch Stanat Slitter machine when performing this service work;     b) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 24 inch Yoder machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 24 inch Yoder machine when performing this service work;       c) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 48 inch Proeco machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 48 inch Proeco machine when performing this service work;     d) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 36 inch Stanat Slitter machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 36 inch Stanat Slitter machine when performing this service work;       e) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 24 inch Paxton machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 24 inch Paxton machine when performing this service work;     f) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247 where the employer did not develop, document and utilized a specific procedure for the 48 inch Wean machine when performing preproduction setup to ensure the employees are properly locking out all energy sources to the 48 inch Wean machine when performing this service work.

The employer failed to ensure appropriate guards were on the coiler and recoiler spoils;    c) This violation occurred on November 13, 2014 at a workplace located at: 811 Regal Row Dallas, TX 75247; where a machine operator was operating the 36 inch Stanat Slitter machine.
