29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Mold Processing: The operator was exposed to continuous noise levels at 67.74% of the permissible 8 hour time weighted average of 85 decibels (dBA). The equivalent dBA level of 67.74% is approximately 87.1dBA. A sampling survey was conducted on November 13, 2014 for approximately 420 minutes. A zero time exposure was assumed for the period of time that was not sampled. b) Clean Room. 2: The operator was exposed to continuous noise levels at 85.02% of the permissible 8 hour time weighted average of 85 decibels (dBA). The equivalent dBA level of 85.02% is approximately 88.8dBA. A sampling survey was conducted on November 13, 2014 for approximately 420 minutes. A zero time exposure was assumed for the period of time that was not sampled. c) Clean Room 3: The machine operator was exposed to continuous noise levels at 60.72% of the permissible 8 hour time weighted average of 85 decibels (dBA). The equivalent dBA level of 60.72% is approximately 86.4dBA. A sampling survey was conducted on November 13, 2014 for approximately 420 minutes. A zero time exposure was assumed for the period of time that was not sampled.
