Each employee engaged in roofing activities, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system: On a flat roof of a commercial shopping center, South side, on or about November 12, 2014, employees without fall protection were exposed to a fall hazard greater than 6 feet to the paved ground below while installing metal coping on the roof edge of the shopping center.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On a flat roof of a commercial shopping center, South side, on or about November 12, 2014, employees without fall protection were exposed to a fall hazard greater than 6 feet to the paved ground below while installing metal coping on the roof edge of the shopping center.
