29 CFR 1926.501(b)(4)(ii): Employees on a walking/working surface were not protected from tripping in or stepping into or through holes.  a) On November 4, 2014, and at times prior thereto employees working on the 2nd floor of a townhome at the Linder Springs Apartment Complex were exposed to an unprotected floor opening at the top landing of the stairway.
